This one-day masterclass aims to provide attendees with a comprehensive understanding of the discharge process, including information and resources to enable the acquired skills and knowledge to be used back in the workplace.

The day is structured to offer a good overview of discharge planning processes geared towards patient safety.

Setting up nurse led discharge in your organisation -

The high impact actions for nursing and midwifery (2010) places a call for action on 8 areas of practice. Ready to go - no delays concentrates on increasing the number of patients in the NHS who could have their discharged managed and led by a nurse or midwife.

This workshop covers key areas of: principles, policy, process, documentation and measuring outcomes.

 

Effective discharge Planning getting it right for everyone -

Why do people stay in hospital longer than necessary?

Why does discharge planning go wrong?

Using the ten steps to effective discharge planning as the basis, the class will examine the responsibilities of ward based staff, the role of specialist discharge planning teams and the relationships between the two. it will consider how to get the best out of multi-disciplinary team working.

 

Why estimate expected date of discharge

The session will focus on active discharge planning process where pathways and models will be explored with the aim of reducing unnecessary length of hospital stay. The enabling factors and barriers discussed with a problem solving approach developed for orchestrating a proactive strategy to discharge planning. Estimating the expected date of discharge on the varied types of admissions - simple & complex cases.
